Fluoride and Your Smile: What Region Patients Need to Know About Changing Policies and Oral Health

Since 2015, the U.S. Department of Health and Human Services (HHS) and the Environmental Protection Agency (EPA) have maintained that fluoride at 0.7 mg/L in community water systems is optimal for preventing tooth decay, balancing benefits against the risk of mild dental fluorosis in children.¹ Meanwhile, the Food and Drug Administration (FDA) regulates bottled water under Title 21 CFR Part 165, limiting added fluoride content to 0.7 mg/L—aligned with the PHS recommendation—to ensure consistent exposure across all water sources.²

Fluoride’s role in oral health remains vital: peer-reviewed studies show that communities with water fluoridation experience 25–35% fewer dental caries compared to non fluoridated areas.³ By strengthening enamel and enhancing remineralization, systemic fluoride works alongside daily brushing and flossing—practices that alone clean only about 60% of the tooth surface.⁴

In 2015, EPA established an enforceable primary maximum contaminant level (MCL) of 4.0 mg/L for fluoride in public water systems to prevent skeletal fluorosis, along with a non-enforceable secondary MCL of 2.0 mg/L aimed at minimizing moderate-to-severe dental fluorosis in children.⁵ These federal standards align with HHS’s optimal 0.7 mg/L guidance and the FDA’s bottled water limit of 0.7 mg/L to balance fluoride’s cavity-preventive benefits against the risk of fluorosis. Several states—including Utah (Effective May 2020) and Florida (Effective July 2025)—have enacted laws that prohibit local governments from adding fluoride to public drinking water, reflecting ongoing debates over community fluoridation.⁶
Continuing optimal oral health in light of these policies involves both community level safeguards and personal routines. Use fluoride toothpaste (1,000–1,500 ppm) at home and consider prescription fluoride rinses or custom trays for high risk individuals. Professional fluoride varnish applications every three to six months further protect enamel, and electric toothbrushes with built in timers help ensure thorough cleaning of all surfaces.
